How your information is handled

This form asks for a tax ID and identity documents because FMCSA requires them on the authority application. Here is exactly where that goes.

  • Documents upload directly to our own server. There is no Google Drive, no Dropbox, no third-party form service in the path.
  • The upload folder is blocked at server level. Files are only reachable from our internal admin panel, behind a login.
  • Your details are used to prepare and submit your filings, and for nothing else. We do not sell, share or resell any of it.
  • Documents are kept while your filing is active and for the record-retention period a carrier file requires. Ask us to delete yours at any point after activation and we will.

Truck Dispatching USA LLC acts as your filing agent under a written agreement. If you would rather not submit a tax ID through a form, call (332) 251-8271 and we will take it by phone instead.

Personal information

Names must match your government ID exactly. A mismatch between your ID, your entity registration and your filing is one of the most common causes of rejection.

On MC numbers

FMCSA retired the standalone MC docket number for new registrations in October 2025. Your USDOT number is now the primary federal identifier and new carriers receive operating authority tied to it. Existing MC numbers remain valid. Some brokers still ask for one during onboarding, and we handle that conversation with them on your behalf.

Your operation

These answers decide which authority type we file and which compliance items you actually need. Filing the wrong authority type means paying the $300 FMCSA fee again, so we get this right before anything is submitted.
